Chapter 4.96 RCW
The Spokane County Claim for Damages Form (PDF) must be presented to the Department of Risk Management within the applicable period of limitations within which an action must be commenced. 

*Prior to completing or submitting a form, please read: Instructions for Completing the Spokane County Claim for Damages Form (PDF).


Different Accident Forms

  • If your claim involves a motor vehicle accident, complete, sign and include the Vehicle Collision form (PDF). 

  • If you are presenting a personal injury claim, complete, sign and include the Medical Release form (PDF). 

  • Mail or Deliver Original claim to Spokane County, Department of Risk Management.
