Description |
Transition Services prepare young adults who are getting ready to exit the public school system. The Transition Coordinator meets with students and families to discuss plans for transition from school to adult life. Information provided to students and families may help them learn about the role of DDD case management, housing opportunities, advocacy, support groups, recreation and health care. Secondary education and employment are priorities for discussion.
The Transition Coordinator may also assist young people with developmental disabilities who are turning 21 to review a wide variety of qualified service providers and may also make referrals for placement in appropriate programs, including the Department of Vocational Rehabilitation (DVR). The Coordinator, working closely with the Case Resource Manager at the Division of Developmental Disabilities, also helps locate and develop resources to meet the individual's needs, which may include counseling, crisis and behavioral intervention, medical and dental services, therapies and other social services and day program services as needed.
The Transition Coordinator works closely with public schools and coordinates and conducts presentations on high school transition topics for families, educators, and interested community members to facilitate job placement for students leaving high school. Resource Fairs planned twice yearly by the Transition Coordinator allow students, families, educators, and the general public to meet with local service providers.
