NAME CHANGES


In Spokane County, Name Changes for Adults (Petitioner must be at least 18 years of age) are filed in District Court and Name Changes for Minors (under 18) are filed in Superior Court. Name change petitions may also be filed and heard in Superior Court by victims of domestic violence wherein the file is sealed due to reasonable fear of safety (Sealed Name Change).

Name Changes for Adults (Petitioner must be at least 18 years of age) - The District Court has name change instructions and forms available on line. The filing fee is $95.00 for each filing plus $5.00 for each additional certified copy of the Order Changing Name.

Name Changes for Minors (under 18) - The Superior Court is the proper court to petition for a name change for a minor (under 18). The Superior Court has minor name change instructions and forms on line. The filing fee is $200.00 (cash or money order only). For certified copies, the charge is $5.00 for the first page and $1.00 for each page thereafter.

To purchase forms in person (hard copy), please contact the Spokane County Bar Association Office. The charge for forms is $5.00.

Sealed Name Change - Name change petitions may be filed and heard in Superior Court when the person desiring a change of his or her name or that of his or her child or ward, is a victim of domestic violence. The name change file is sealed due to reasonable fear for the petitioner's safety or that of his or her child or ward.

In Spokane County, the Domestic Violence Consortium, a private, nonprofit organization provides assistance and forms for sealed name changes. The advocates at SCDVC provide a wide range of services to assist any person who is a victim of intimate partner violence or fears violence may happen.

Birth Certificate Change - Once the name change has been approved, if you wish to change the birth certificate (and the person was born in Washington State) you will need to send certified copies of all completed paperwork to the Department of Health. The fee is $13.00 for each new certificate plus the enclosure of a self-addressed, stamped envelope for return.
