Apply to be a Treatment Provider

The Substance Abuse Treatment Division has an open application process for certified treatment agencies who want to contract with us to provide treatment for low income clients.  Applications are accepted year round.

Once your application is accepted, we review the documentation you provide and we will schedule an onsite visit.  The onsite visit is an opportunity for the prospective provider to ask questions about the requirements and responsibilities of contracted providers and for us to see the site and review policies, procedures, and fiscal capability. After the onsite visit, the prospective provider receives a letter advising of our decision.  If a provider is accepted, that provider will receive direct notice of any funding and contract opportunities as they come available.  This gives the agency an opportunity to review and respond to Requests for Interested Parties related to specific contracting opportunities.
